Did you know you can now book to see a GP in Tameside at evenings and weekend? There will be additional appointments available 6.30pm-8.00pm weekdays and 9am - 12pm weekends. Appointments must be booked in advance. Please speak to the receptionist to book an appointment or for more information.

Patient Participation Group

We now run a Patient Participation Group at the Practice.

If you have time to give and would be interested in the involvement of improving our Services, then we would like to hear from you.

Please leave your details with one of our receptionists and we will get back to you in due course.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Address

Please notify the practice if you change your address or telephone number as soon as possible. It is important to have the correct details in case we need to contact you. Patients who move outside the designated practice area will be required to register with a doctor in their own area.
